Annual Health and Medical Record Form
The 2011 update to the Annual Health and Medical Record consolidates all records from the National Council and the national high-adventure bases, enabling a single form to be used for all BSA activities. The new form, bin No. 680-001, replaces the previous, catalog No. 34605, and is available by clicking this link.
